All exams will be proctored on the date listed.
Applications are due as required below. Please send the original form and fees to BSDW's Operator Certification Program and keep a copy of the application and attachments for future reference.
Examinees will be contacted by a proctor with testing schedule details and logistics.
Test Dates
- September 23, 2009 (application deadline has passed)
- March 12, 2010 (applications due by 5:00 p.m. on January 26, 2010)
Please follow all instructions on the form and ensure your application is complete, with any necessary attachments. Applications and fees must be received at the NDEP's Carson City office by 5:00 pm on the application deadine date to be considered for the exam.
Please indicate preferred test site on your application: Battle Mountain, Dayton, Elko, Ely, Hawthorne, Henderson, Las Vegas, LVVWD, North Las Vegas, NRWA Conference, Reno, TMWA, West Wendover or Winnemucca.
