The following is a listing of field names used by different query reports for Nevada Division of
Environmental Protection (NDEP), Bureau of Corrective Actions, .dbf files:

Nevada - Registered Tank Information:
     AltFacilityID  Facility ID Number
     TankID         Number assigned to a specific tank at a facility (i.e., 1, 2, 3, ...)
     TankStatusDesc Tank status (e.g., Active, Temporary Closure, Permanent Closure)
     TankCapacity   Capacity of tank in gallons
     SubstanceDesc  Description of substance in tank (e.g., diesel, gasoline, heating oil,
                    etc.)
     TankMatDesc    Material of which the tank is composed (i.e., steel, fiberglass, etc.)
     TankModsDesc   Any upgrades of tank material (i.e., cathodic protection, coating,
                    etc.)
     PipeMatDesc    Material of which the piping is composed (i.e., steel, fiberglass,
                         etc.)
     TankModsDesc   Any upgrades of piping material (i.e., cathodic protection, coating,
                    etc.)
     DateIntalled   Installation date of tank

Nevada - Owner Information:
     AltFacilityID  Facility ID Number
     Name           Name of facility owner
     Address        Street address of facility owner
     City           City of facility owner
     State          State of facility owner
     ZIP            ZIP code of facility owner
     Phone          Phone number of facility owner
     OwnerDisc      Description of facility owner (e.g., private, government, etc.)

     site_id        facility ID number
     county         county of facility
     activ_id       activity event number = indicates a discrete release at a particular
                         facility
     activ_type     type of activity. Can be: Investigation, Intend to Close, or
                         Confirmed Release
     rep_date       Date release reported to NDEP
     pet_fun_id     petroleum fund ID number (used to track claims for
                         reimbursement)
     dem_number     State Division of Emergency Management tracking number
     officer        NDEP case officer working on case
     fil_loc        location of paper file (Carson City, Las Vegas, Washoe County
                         Health Dept., Clark County Health District)
     media          type of media impacted (soil, water, etc.)
     contain        type of container from which the release occurred (e.g., UST, AST,
                    other, etc.)
     contam         substance released (e.g., gasoline, TCE, etc.)
     contam_des     description of contaminant
     la_date        date of last action
     la1            last action memo field
     la2            last action memo field
     na_date        date of next action expected
     na1            next action memo field
     na2            next action memo field
     c_date         date of closure
     c_type         regulatory type of closure
     company        company of Nevada Certified Manager conducting clean-up
