Information Required: An application provided by the division must be completed. The following must also be submitted: a color photograph; proof of at least one year's experience and of participation in fifty tests of UST's; proof of training by the testing equipment manufacturer; proof of completion of a division-approved course on safe handling of UST's; a statement declaring the details of all pleas of guilty or nolo contendere in any criminal proceeding and of any conviction of a crimes. Also needed are three letters of recommendation and proof of passing the required examination given by the ICC.
Applicant is required to show proof of passing the required examination given by the ICC on the Tank Tightness Module.
